Position Summary: Reporting to the Health Director, the Chronic Disease Management (CDM) Nurse will provide preventative and chronic disease management programs primarily for metabolic, cardiac and pulmonary patients. The CDM Nurse will provide direct patient care and be a resource to the primary care providers, other allied health professionals, community healthcare partners, and community members. The CDM Nurse will participate in the development, implementation, and evaluation of existing and new programs and strategies to improve how we offer care to our community. The CDM Nurse will also participate in program development and will provide mentorship to students completing placements at the Health Centre.
