Our Team

The Primary Health Care Team provides community-based health services that support the physical, mental, and emotional well-being of Saugeen First Nation community members. Our team works together to provide culturally safe, holistic, and person-centered care that supports individuals and families across the lifespan.

The Primary Health Care Team currently includes:

  • Family Physician
  • Nurse Practitioner
  • Occupational Therapist & Psychotherapist
  • RPN & Foot Care Nurse
  • Phlebologist
  • Quality Improvement & Decision Support Specialist (QIDSS)

Services Provided

The Primary Health Care Team offers a variety of health services and supports including:

  • Primary medical care
  • Chronic disease management
  • Preventative health care and wellness support
  • Mental health and emotional wellness support
  • Occupational therapy services
  • Foot care services
  • On site blood lab services
  • Health system navigation and referrals
  • Care coordination and community partnerships
  • Quality improvement initiatives that support enhanced client care and health outcomes

Our Approach

Our team is committed to providing compassionate, culturally respectful, and community-centered care. We work collaboratively with individuals, families, community programs, and external health partners to support holistic wellness and improve access to care within the community.

Access to Care

SFN Members and families living within the community or off-reserve are welcome to access services provided by the Primary Health Care Team. Eligible community members and families may also roster with our Family Physician and Nurse Practitioner to support ongoing access to primary care services, continuity of care, and comprehensive health support tailored to individual and family needs.
